Short answer: Telehealth testosterone therapy is legal in Washington if your provider holds a Washington license, and Washington removed the out-of-state pathway in 2024, so that is not optional. For soldiers at Joint Base Lewis-McChord, the Army's aeromedical rules are the most workable of any service. For anyone on a boat out of Kitsap, they are the strictest.
Start with what you hold, not how you feel
JBLM covers I Corps, special forces, and an Air Force airlift wing, with Kitsap, Bremerton and Whidbey inside an easy drive. Which community you are in changes the answer more than your symptoms do.
Army aviation. Waiverable, and routinely waived. Temporary clearance comes once you are stable on a single approved medication with no significant side effects. The diagnosis has to be made with two or more early-morning low draws plus symptoms. Annual renewal wants total testosterone in the 400 to 800 range, drawn at the midpoint between injections. Only monotherapy will be considered, clomiphene is prohibited in aviators, and locally compounded formulations are prohibited outright. That last one matters when you are comparing cheap online clinics, because many of them run on compounded product.
Submarine, dive, or special warfare. Ongoing exogenous testosterone is disqualifying under Navy medical standards, with no waiver path in the manual. If you are on a boat out of Bangor or Bremerton, this is the rule that governs your decision.
Air Force aircrew at McChord. Injectable hormone replacement is disqualifying for flying duties and for retention. Topicals are disqualifying for flying duties. Waivers exist and are demanding, and men who do not meet the diagnostic criteria in the first place are unlikely to get one.
Everyone else. The new DoD guidance says plainly that neither a diagnosis nor stable maintenance therapy is, by itself, a basis for a duty limitation.
The workup has to come before the treatment
Every one of those waiver paths requires two or three early-morning, fasting, low draws taken before any treatment, plus LH and FSH.
Start at a cash-pay clinic first and that evidence no longer exists. Rebuilding it means stopping and waiting for your own axis to recover, which can take a long time. This is the single most expensive mistake available to you here, and it is the one the cheap clinics encourage.
The winter question, honestly
Tacoma gets around eight and a half hours of daylight at the solstice, with a long grey stretch either side. Low energy, flat mood, worse sleep and harder training are the exact symptom list people bring to a hormone clinic, and every one of them has other explanations here.
DoD's own guidance says a specimen does not count if it was drawn within 30 days of training involving extreme physical or mental stress or sleep deprivation. Field problems and selection courses tank testosterone temporarily. Marines through a seven-day SERE course dropped about 44 percent. Soldiers in an eight-day field exercise dropped about 28 percent and recovered within four days.
Come out of the field in February and get one afternoon lab, and you will look hypogonadal when you are not.
If you are active duty
Care starts with your primary care manager. TRICARE requires it and there is no point-of-service option, so a civilian clinic is out of pocket.
You also have a standing duty to report significant health information to your chain and to authorize civilian providers to share records with the military health system. Plan on your command knowing.
If you are getting out
JBLM separates a lot of people, and the gap is the thing to plan. You have 90 days after a retirement date to enroll in a TRICARE plan. The transitional program people mention is for involuntary separations, not ordinary retirement. And there is no VA rating code for low testosterone, so a claim has to be built on residuals or a secondary connection rather than on the words themselves.
Questions people actually ask
Does my provider need a Washington license? Yes. Washington removed the out-of-state telehealth registration pathway when it adopted the Uniform Telehealth Act in June 2024.
Can Army aviators be on TRT? Yes, with a waiver, once stable on monotherapy. Compounded formulations and clomiphene are prohibited.
What about submarines? Ongoing exogenous testosterone is disqualifying for submarine, dive and special warfare duty.
Is a questionnaire enough to get a prescription? No. Washington's medical commission has been explicit that an online questionnaire is not an acceptable medical interview for prescribing.
Should I get tested right after a field problem? No. DoD's own guidance invalidates specimens drawn within 30 days of high-stress or sleep-deprived training.
